Cause Description:
Center for Elders Independence (CEI)is a 501(c)(3) non-profit Program of All-inclusive Care for the Elderly (PACE) which helps frail elders aged 55 and over, who are at risk for nursing home placement stay in their home and the community. We do this by providing comprehensive medical and social services, a PACE Day Center, home care, transportation, meals...anything and everything an individual might need to remain living at home safely and securely. All services are managed and coordinated by an interdisciplinary team made up of Physician, Nurse Practitioner, RN's, Social Workers, Occupational, Physical, & Recreational Therapists, Transportation, Home Care and PACE Center staff.CEI serves an exclusively frail population: California’s most vulnerable seniors. PACE integrates a full range of services for a higher quality of care. CEI’s mission is to provide affordable, integrated health care services to the elderly, which promote autonomy, quality of life and the ability of individuals to live in their community.
